Forms For Campers
 
Eligibility: Our camp is co-educational (for boys and girls) ages 9-15 only.  Your camper must be age 9 by the start date of camp and we cannot make any exceptions.  Thank you for your understanding with this requirement.
 
Tuition: The cost for one camper to attend Masse Moro is $600.00, without lodge support.  The application fee of $50.00 is due upon submital of your camper application and is credited to the $600.00 tuition.  While the application fee is due with the application, the final tuition balance can be paid at any time, and is due by June 15th of the camp year.  We accept checks, money orders or cashier's checks, and online payment with a credit card or personal bank account.  For further information about the payment of fees, please Click Here "Payment" tab
Camper Application Forms (Select Your District)
 
If you are between the ages of 9 and 15 and are interested in attending our camp, please follow these four easy steps for submitting your application! 
 
  • First, find out which district you live in by reviewing the DISTRICT MAP.  You can also locate a local lodge by clicking on the link or pasting the path into your web browser:

  • Second, click on the application that corresponds with the District you live in.  Fill out the entire application and submit completed forms according to their instruction.

 

  • Third, find out whether or not you are elligible for a scholarship!  Contact a member of your local lodge as early as you can!  See Q/A under "Frequently Asked Questions" for more information. 

 

  • *If you live in District Five, please download the "District 5 Matching Funds Form" and follow instructions for receiving matching funds from the District! 

 

  • Fourth, submit your health form - see instructions to the right of this field.

 

 

Camper Health Forms

Due by 7/1 in any camp year - please mail no later than July 15 to ensure time to arrive before camp starts.

 

There are two health forms, found here: 

1.  Health Form 1: Camper Medical History

ATTENTION Parents/Guardians - please schedule an appointment with your medical provider as soon as you are able and be sure to bring the health forms along for the appointment. 

 

Instructions for the two Health Forms are outlined below - Click the title of forms (above in blue) to open the form in a new window, print both.  

 

Form 1: Parents are asked to complete pages 1-3 of this form, leaving the forth page blank for our camp health care personnel upon your child's arrival at camp.  This will allow for better pre-camp screening and documentation.

 

Form 2: When your child goes to the doctor for their appointment, please have the medical personnel complete this form for you.

     

Please staple the 2 completed forms together at the upper left hand corner and mail them both at the same time in one envelope to assist in the filing process.  Please do not three hole punch your forms.  You are welcome to send an electronic copy of your forms to save on postage to:
